Letter of Permission

We can only issue a letter of permission for verified registered, active members of St. Mark the Evangelist Catholic Church. If the parents/guardian wish to have their child’s sacrament/event celebrated at a Catholic church other than St. Mark they must fill out and submit the form. Please allow 10 business days to process your submitted request.
